In consideration of my child's participation in Lax Elite Camp ("the Camp"), I, the undersigned parent or legal guardian, acknowledge and agree to the following on behalf of myself and my child:
Assumption of Risk. I understand that lacrosse and related athletic activities involve inherent risks of physical injury, including but not limited to contact, collisions, falls, and equipment use. I knowingly and voluntarily assume all such risks, whether known or unknown.
Release of Liability. I hereby release, waive, and discharge Lax Elite Camp, its coaches, staff, volunteers, guest coaches, and organizers from any and all claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child while participating in the Camp.
Fitness & Emergency Care. I affirm my child is physically able to participate. In the event of injury or illness, I authorize Camp staff to secure appropriate emergency medical treatment.
Media. I grant permission for photographs or video taken at the Camp to be used for promotional purposes.
